Lightpath provides complete, flexible care for chronic conditions offered as multiple tiers of support — or program intensities — based on a person's health needs, such as the severity of their condition(s). As clinically indicated, members will be able to “move” from more to less intense programs to help them achieve and maintain their unique goals.
Lightpath matches members with the right care pathway and clinical expertise for comprehensive support during and after GLP-1 use. This includes a clinical team that helps address barriers to outcomes, like side effects and adherence, access, and nutrition.
Metabolic Intensive focuses on members with type 1 diabetes, type 2 diabetes with an A1c > 8% or hypertension with blood pressure > 130/80, inclusive of comorbidities. Members will have access to a dedicated team of licensed experts in metabolic health, nutritional support, behavioral wellness, precision prescribing, and medication adherence as clinically indicated, and paired with AI-enabled support. Plus, medical provider coverage is in all 50 states.

With GLP-1 use, the learning platform will inform the right GLP-1 utilization and clinical care for promoting better health and lower costs.
