Your members are unique. Their care should be, too.
Lightpath draws from Verily’s deep clinical expertise, as well leadership in health tech, healthcare regulation, and data science to enhance clinical care with AI. Lightpath will offer your plan members more accessible, timely, tailored and engaging metabolic care to support better program adherence, outcomes and costs.
A flexible, multi-tiered solution*
Help more members meet and maintain health goals
Lightpath Metabolic provides comprehensive and flexible metabolic care, offering support for multiple tiers of acuity. As clinically indicated, members will be able to move from intensive programs to less intensive programs to help achieve and maintain their metabolic health goals (see Figure 1 below).
Lightpath matches members with the right care pathway and clinical expertise for comprehensive support during and after GLP-1 use. This includes a clinical team that helps address barriers to outcomes, like side effects and adherence, access, and nutrition.
Metabolic Intensive focuses on members with uncontrolled type 1 and 2 diabetes (A1C of 8.0% or greater), inclusive of comorbidities. Members will have access to health coaches, physicians, pharmacists and registered dietitians, as clinically indicated.**
Evidence-based care
Verily Onduo clinical quality
Proven chronic care outcomes
Lightpath is an evolution of Verily Onduo virtual care management for chronic conditions, building off its best practices and learnings, and designed to be reflective of its successes, such as demonstrated outcomes. A peer-reviewed, 4-month study of Verily Onduo with intermittent CGM use in adults with type 2 diabetes showed meaningful glycemic improvements. Other improvements in metabolic health parameters were also observed, including weight‡, total cholesterol and blood pressure.1
Image: 2.4% A1C decrease in high-risk participants (avg.), 2.4 hours per day increase in time in target blood glucose range (avg.), 4.4 mmHg reduction in systolic blood pressure (total, avg.), 16.6 mg/dL decrease in total cholesterol (avg.) and 9 pound weight loss (avg.). Note: Weight loss results are not guaranteed. Individual weight loss outcomes, amount and time duration will vary.
A smarter, personalized approach*
The right care, at the right time
Members receive tailored nutrition content, nudges and interventions, as well as holistic medication services, including support for GLP-1s and other anti-obesity medications.†
As an evidence-based, learning health system model, Lightpath will continually improve care quality and patient outcomes. With GLP-1 use, the learning platform will inform the right GLP-1 utilization and clinical care for promoting better health and lower costs. This combined with an ongoing, comprehensive care approach and expert clinical team means members can overcome barriers to care for sustainable progress.
