¹ Verily Health Inc. (“Verily”) offers virtual care management programs for eligible individuals, as further described in these materials and at 

. Verily collaborates with Onduo Management Services LLC (“OMS”), Onduo LLC,  and a network of affiliated Professional Entities to offer the services. These services are meant to be used in conjunction with regular in-person clinical services and not intended to replace routine primary care.  


² The Verily Type 1 Diabetes Program is not intended as a substitute for your primary care physician’s guidance. Before making major lifestyle changes, please discuss your treatment plan with your primary care provider.


³ There is no cost for most health plan members. If you have a qualified high deductible plan, you may have to pay out of pocket for some services with this solution until you meet your deductible. To determine if you have any costs for care, you can call the Member Service team at the number on the back of your ID card. If you are eligible, you will receive an invitation to enroll. Eligibility is determined by several factors including your medical status, health plan coverage and historical medical claims. If you are not initially able to enroll, you may be able to upon meeting eligibility criteria.


The Verily Me Software is intended to be used by the following adults aged 18 or older on web and iOS and Android mobile applications (1) users to enroll, track and self-manage their health and receive educational/wellness information and (2) caregivers to enroll, track and manage health and receive educational/wellness information on participants who are 13-17 years of age. 


The Verily Diabetes Program is not intended to support individuals who have not been diagnosed with type 1 diabetes, are pregnant, have an organ transplant history of any kind, have a heart condition requiring a ventricular assist device (e.g., LVAD), are living with any of the following: severe or end stage kidney failure, cirrhosis or liver failure, dementia (e.g., Alzheimer’s disease), or cystic fibrosis.
